When your chaperone and student roster is determined, please fill out the registration form online (below) with names of chaperones and youth attending. This must be done prior to February 17, 2023. In order to submit the form below, please note that the "I Agree" box has to be checked on all 8 chaperone boxes in order for it to go through, even if you don't have that many chaperones. The form will not Submit otherwise, we appreciate your attention to this. This way chaperones can be verified by the diocese prior to having all the paperwork completed. The forms listed here should be printed and completed by the appropriate people. Be sure and set a reasonable timeline for all paperwork to be returned to the Group Leader so that there is time for their review as well as to be sent in by February 17, 2023. Please note, any youth whose forms are not complete will not be allowed to stay at the event. Parishes will be billed $120 per participant based on the online registration forms submitted online by February 17th. No refunds will be allowed after the February 17th deadline.
